Summary:

INTRODUCTION: Acute acalculous cholecystitis represent about 5-10% of all cases of acute cholecystitis. It occurs more often in hospitalized and immunodeficient patients.

THE AIM is to highlight the diagnostic importance of ultrasound like fast, non-invasive procedures and importance of repeating it in patients with acute abdominal pain.

METHODS AND MATERIALS: Observational protocol of patients from the Emergency medical Service Niš, discharge papers of Surgery clinics. In case report of a 45-year-old male, a rare case of acute acalculous cholecystitis is presented in the beginning with delay in diagnosis, but with a good outcome.

DISCUSSION AND CONCLUSION: Postponement of diagnosis in very rare acalculous cholecystitis increases mortality rate, because it is not often recognized at early stage due to the absence of gallstones. Ultrasound is the method of choice for diagnosing a disease with high mortality rate.

Keywords: acalculous cholecystitis, high mortality, ultrasound.
